Project Background
Health Data Together supports community engagement throughout Pierce County’s community health planning process. Earlier phases focused on gathering community input to improve how health equity indicators are selected, grouped, and used.
That phase of work is complete, and the Community Health Assessment is now in communications review. Findings from this work are informing the next phase, the Community Health Improvement Plan, which will focus on translating assessment insights into shared priorities, strategies, and actions to improve health and equity across Pierce County.
This site will continue to be used to provide updates, context, and opportunities for engagement as the planning process moves forward. Questions about this work can be directed to chip@tpchd.org
Health Data Together brings the community together to improve health data. By listening to your feedback and ideas, we want to make sure this data is accurate and useful. We believe that by understanding real-life experiences and views from our community, we can make health info better and more effective for everyone.
Synthesize Community Health Assessment findings: Integrate quantitative data, community partner input, and lived experience to describe key health challenges and assets across Pierce County.
Engage partners in interpretation: Create opportunities for community partners and residents to reflect on assessment findings and help inform priority areas.
Support the transition to improvement planning: Use assessment insights to inform the shift from data collection to Community Health Improvement Planning.
Maintain an ongoing feedback loop: Use this platform to share updates, clarify where the process stands, and invite continued community engagement over time.
